BMI predicts population-level health outcomes — but at the individual level, its predictive accuracy for metabolic disease is substantially lower than most people assume, and newer research using large datasets and machine learning has identified the specific combinations of measurements that predict cardiometabolic risk far better than BMI alone

This article addresses BMI and metabolic syndrome — specifically the cluster of risk factors that better predict cardiovascular disease and type 2 diabetes than BMI, and what current guidelines actually recommend for risk assessment.


Metabolic syndrome: the cluster that predicts risk better than BMI

Metabolic syndrome is a cluster of conditions that, when occurring together, dramatically increase the risk of cardiovascular disease, type 2 diabetes, and stroke. The diagnostic criteria (using the 2009 Joint Scientific Statement harmonised criteria) require any three of the following five:

  1. Waist circumference: ≥94 cm (men) or ≥80 cm (women) — with lower thresholds for Asian populations
  2. Elevated triglycerides: ≥1.7 mmol/L (150 mg/dL) — or being treated for elevated triglycerides
  3. Reduced HDL cholesterol: <1.0 mmol/L (40 mg/dL) in men or <1.3 mmol/L (50 mg/dL) in women — or being treated for low HDL
  4. Elevated blood pressure: ≥130/85 mmHg — or being treated for hypertension
  5. Elevated fasting glucose: ≥5.6 mmol/L (100 mg/dL) — or being treated for type 2 diabetes

Why this is more predictive than BMI alone: metabolic syndrome captures metabolic dysfunction (actual changes in lipid metabolism, insulin sensitivity, and blood pressure), while BMI captures only the weight-to-height ratio, which correlates imperfectly with metabolic health.

The TOFI population (Thin Outside, Fat Inside — individuals with normal BMI but significant visceral fat and metabolic syndrome) are misclassified by BMI as "not at risk" while having the same or higher cardiometabolic risk as obese individuals by BMI criteria.


A Body Shape Index (ABSI): beyond BMI and waist circumference alone

A Body Shape Index (ABSI), developed by Krakauer and Krakauer (2012), attempts to capture the additional risk from abdominal obesity beyond what weight and height explain:

ABSI = waist circumference / (BMI^(2/3) × height^(1/2))

The formula normalizes waist circumference for height and weight, isolating the abdominal component of body shape. Higher ABSI indicates greater abdominal adiposity relative to overall body size.

What makes ABSI different from waist circumference alone: two people with the same waist circumference but different heights and weights have different ABSI values — a 90 cm waist is more concerning on a shorter, lighter person than on a taller, heavier person, and ABSI captures this.

Predictive validity: studies have shown ABSI is a better predictor of all-cause mortality than BMI alone, and adding ABSI to BMI improves risk prediction compared to BMI alone. It's not yet widely used clinically (waist circumference alone is simpler to measure and communicate) but represents the direction of research.


Waist-to-height ratio: the emerging clinical recommendation

WHtR (Waist-to-Height Ratio) = waist circumference ÷ height (in the same unit)

The 0.5 threshold: "keep your waist to less than half your height" is the practical guideline emerging from the research. A WHtR below 0.5 is associated with lower cardiometabolic risk across sexes and ethnicities — a universal threshold that BMI's sex- and ethnicity-specific thresholds don't provide.

Why it beats waist circumference alone: waist circumference requires sex- and ethnicity-adjusted reference values (94 cm for European men, 90 cm for South Asian men, 80 cm for all women). WHtR uses a single threshold (0.5) because taller people are expected to have proportionally larger waists.

Current clinical adoption: the Royal College of Physicians (UK), NICE, and several international bodies now recommend measuring waist-to-height ratio alongside BMI in clinical settings. The "BMI plus waist circumference" recommendation is gradually evolving toward "BMI plus WHtR" as the evidence base strengthens.


BMI and all-cause mortality: the "obesity paradox"

The "obesity paradox" is a repeatedly-observed phenomenon in certain clinical populations where overweight and mildly obese individuals (BMI 25-35) have better survival outcomes than normal-weight individuals with the same diagnosed condition:

In heart failure: multiple studies show that heart failure patients with BMI 25-35 have better survival than those with BMI <25.

In chronic kidney disease: similarly, slightly elevated BMI is associated with better outcomes.

Proposed explanations:

  • Reverse causality: serious illness causes weight loss, so normal BMI in a sick population may indicate longer-established disease severity
  • Metabolic reserve: in conditions with high caloric demand (heart failure, cancer), additional body mass provides reserves that improve survival
  • BMI's body composition limitation: normal-weight individuals with low muscle mass and high fat ("sarcopenic obesity") are misclassified as healthy by BMI

The practical message: BMI is least reliable as a health indicator in specific clinical populations with established disease — its population-level associations with risk are strongest in asymptomatic general populations, not in people already managing serious conditions.
